Community Living Services Annual Report

Hello Friends and Family,

Our goal in the Residential Services department is to not only promote independence and to assist individuals in achieving their goals, but to be a companion as well.

We currently operate three residential group homes which are licensed by the Indiana State Department of Health and funded through Medicaid. We currently serve 22  consumers in our group homes . The Direct Care staff provide supports with adult living skills, community integration, and health and safety to the individuals we serve.

This year, two consumers from the group homes received referrals for the waiver program.

The residents participated in the Polar Plunge, Special Olympics, walked in local parades, helped with the community wide Clean Sweep program and Meet Your Candidate which resulted with several consumers registering and voting. Residents also entered the Knight’s Of Columbus fishing tournament for individuals with disabilities, walked in the Jennifer Riley 5k, Pam Mair and Julie Brannon  carried the torch for the state of Indiana, and other activities through out the year.

Residents work hard every year to achieve their goals. They look forward to the annual banquet and to receive  awards and acknowledgements for their successes.
